Help- tiger in trouble???

Any help would be appreciated.......
I've had my tiger salamander (wild) for almost 8 years now and never had a problem with his eating or shedding. About three weeks ago he stopped eating and had what i though was still some skin stuck to him after shedding. You can see the state he is in by the pictures.

I don't know what it might be, I always used tap water in his water dish and never had a problem. I just redid his tank with dirt and moss about 8 months ago. He's getting really skinny and weak. I've always fed him mealworms from the bait shop.
His home looks like the following:

Going to try trout worms and redo his tank with (zoo-med)sphagnum moss. Any other help???
