Help!!!

Hi, i am in need of help, my axolotl is not looking good, she has lost wieght and when she swims she is spinning, and when resting is tilting to one side, she has been like this for a couple of days now. All help and advice will be greatly appreciated!!!
 
Can you tell us more about her(age/size)? what is she fed and how often? and also her tank - size, frequency of water changes, what the ammonia/nitrite/nitrate readings are? Substrate(sand, gravel, etc.)? Temperature of your tank? A picture would also be helpful.
