Axolotl looks like he is coughing/gagging

So my axolotl had a fungus infection about a week ago and since then he is doing a lot better and eating again. His gills basically fell off but since treating him daily (which has been documented thoroughly in my last post) he has looked a lot better and appears to be on the mend. He is still in the hospital tank with dechlorinated water and daily water changes.

Recently though, he has been doing this odd thing where he looks like he is coughing or gagging whenever I change the water and now when I feed him. He's been getting a single one inch piece of nightcrawler since he started eating again. He has ate successfully 4 times since being sick. He also does this after coming up to the surface for air. He tends to do it a lot during water changes then he stops even though I do everything possible to acclimate him. He also kind of squirms backwards and doesn't look comfortable when this happens. The other thing I noticed is I have not seen a bowel movement in a few days. I am not sure if that might have to do with this either.

Can someone try to explain this behavior to me? I've had him for 6 months and have never experienced this.
 
how is the water change performed ie.. axie removed and 100% change, axie in tank 50% change 2x daily, etc..
what dechlorinator are you using?
does he have a hide/cave in the hospital tank.
are the worm bits still wriggling when they are given?
is 3g per litre still being used, have you been able to source the other ingredients?
