Which species

I got what was ordered as a baby two toed amphiuma and the animal itself look like one, same head shape, two toned coloration etc... But what confuses me is that this particular specimen has two toes on the back legs but only one on the front legs. It does not look like a pholeter (wrong head structure). Is this possible that it's just a mutant or damaged two toes? Or do pholeter have two toes on hind legs.
 
It's in a temporary setup while I figure out what it is.
