Dead axolotl - Q on the cause

Hmm...impacted eggs maybe? I've heard of it in axolotls, but never heard symptoms described, so I'm not sure.
 
One of my axolotl's died a few months ago with a few similar symptoms. She had a swollen jaw and her mouth was gaping open, and her gills were also curled forward. She also seemed normal before she died apart from her stopping eating. But I don't recall her cloaca being swollen or anything so maybe it was something completely different. This is probably no help though anyway because I'm not sure why she died.
 
As I understand it, swollen throat region in an axolotl is heart failure. I'm no expert, though.
 
How do they get heart conditions that would lead to a heart failure?
