A grotto salamander...I know the photo has some very weird lighting going on, but they're tough to get good lighting on without really disturbing them.
it's Eurycea spelaeus (formerly Typhlotriton). Tim, that is skin covering the eye and they are blind to various degrees. All of mine still seem able to detect light if you shine it on them.
