Help, Axolotl toes missing with red worms in tank!!

Hello everyone. I have an issue in my tank that im not quite sure how to handle. I've notice recently that I have lots of red ish worms in my tank. The worms seem to hide when my filter is on and come out when I feed the axolotls. Problem two is I noticed that the Axolotl toes are damaged as if they were eaten? Is it because of the worms? Otherwise that seem healthy and normal. I have checked ammonia and nitrate levels and there fine. Any advice welcomed. I have been changing my water about twice a week. I do have sand in the bottom which im kinda of regretting now. I did neglect cleaning the debris in bottom from plants do to the fact that I have a newborn. (Trying to be more aggressive about it now).

You have planarian i have them in my tanks. They should be harmless i have them in a 5 gallon tank that has two 1 1/2 inch c oriental in it. There almost impossible to get ride of. You can cut them up into little pieces and all the pieces will come to life and be little planaria. If you want to eradicate them its easier to buy new aquarium and supplies.
 
Planaria. They are common.

Keep the water cool and clean for the toes and make sure he's not cutting or rubbing them raw on anything. Watch for fungus.
