Is he sick?

I’m not sure if anything’s up with my axolotl. He was staying in place for a while when I went to feed him before going to bed. I put the food in and waited around half an hour. Usually by then he eats up, but this time he just l stayed in place. I turned on the light to see if he was sleeping and he didn’t really react. I gently moved him to the food and he didn’t eat it. Now he’s staying in place in another part of the tank. Some issues that may be prevalent are high nitrates and my friend was keeping him for a week or so last week, I showed him how to tub him. Do you guys think he’s just being lazy or is something wrong? Ammonia and nitrates were both 0. Also, his weird gill has pretty much always looked like that. I also looked closer at his eye and it looks kinda like a ring, shown in the second picture.

Update: it’s been a bit and I put my hand in and he started biting it like usual. He’s starting to move more too. Also I’m wondering if my substrate is good. It’s called Stoney River premium aquatic substrate and it’s black sand. The place I got him at had one passionate worker and he kept all of his Axolotls in the same substrate. If it’s dangerous for the axolotl I’ll change it but I would only want to do so if absolutely necessary. Also the axolotl has packed on a few pounds so maybe he’s impacted? Idk, he’s still eating every other day so far. Any thoughts would help!
 
They are nocturnal so he was just probably waking up. You mentioned you friend kept him for a week so he probably just was re-acclimating too.

he's probably just sleepy, they can be quite lazy at times.
