I can't imagine that a nitrate level of 40 would cause that problem, otherwise my axolotls would be bald because my tapwater has nitrate levels of 40.
Hopefully one of the more experienced keepers will be able to help.
If necessary you may have to start another thread in the sick axolotl section just for the gills thing.
Sorry I can't be of more help, but I don't have a huge amount of experience.
I guess you could always search through the sick axolotl section to see if anyone has had a similar problem with their axolotl.
