Illness/Sickness: What is this fungus, worm???

My GFP Hulk is losing his gills. There is a white looking thing on a couple of his gills. Can someone tell me what it is and how to remove it??

The white parts are scar tissue, not worms or fungus. What do you think caused him to lose his gills like this? Better question, what are your water parameters? What is your tank setup like? Has he had problems like this before? Does he have any tank mates?
 
It was poor water quality but I fixed that. Third pic is his tank buddy, her gills are fine. I have been changing 20% water regularly, water temp is ok. It got over 95 here- temp went up on the tank a bit. I was concerned because it was not going away and the gills need to grow back.
 
His gills we grow back eventually. If you want to help him along, you could give him a tea bath every once in a while. It will help soothe the skin and aid in regrowth.
